The coin was issued during the reign of Macrinus (217-218 AD), who was a Roman usurper, and it bears his image along with that of his predecessor, Caracalla (211-217 AD). This is unusual because Macrinus was not a legitimate successor of Caracalla, and it is believed that this coin was minted to legitimize Macrinus' rule and connect him to the previous emperor.
