Van Gelder & Hoc (p. 84) call this an enseigne of the gold real which appears to mean a large pattern.  It is nearly double the size (hence, not a piedfort), and nearly 6 times the weight of a normal real.   Its purpose is not known.

One interesting fact about the Pattern Gold Real (Large pattern) - Charles V ND (1543-1555) from Spanish Netherlands (Duchy of Guelders, Dutch States) made of Gold weighing 30.58 g is that it features a unique pattern, known as the "Large Pattern" or "Großmünz" design, which was used for a short period of time during Charles V's reign. This pattern includes a larger-than-usual shield on the obverse (front) side of the coin, surrounded by a wreath of leaves and flowers. The reverse (back) side of the coin features the imperial eagle, also surrounded by a wreath. This design was used to showcase the power and wealth of the Spanish Empire during Charles V's reign.
